索取入學與獎學金表格申請書信範例
索取入學及獎學金表格是申請程式的第一步，這一步比較簡單容易。只要學校初選名單確定下來，便可
發函索取表格。索取申請表格的信函格式與商業書信大致相同。這類申請信函最好寄到位，而不要寫給
個人，以免因工作調動而耽擱。以美國大學為例，申請的受理單位可能有以下幾種：
1.由Office of Admissions統一受理一切入學申請函件，包括大學部與研究生院；
2.由Office of the Registrar受理，許可權同1；
3.由Office of Student Service受理，許可權同1；
4.由Office of Foreign Student Advisor受理國外學生入學申請函件；
5.由Office of Foreign Admissions或Office of International Scholars受理國外學生入學申請
函件；
6.由Graduate School, Graduate College或Office of Graduate Studies受理研究生院的入學申請
函件；而Office of Admissions受理本科部的申請；
7.由各所屬分院（如College of Eng.)分別受理入學申請信件，也有很多學校先由Office of
Admissions接受申請，供給所需表格，然後一切通信改由各分院負責，大的學校大多採用這種方式，
因為他們每年受理的申請函件高達上萬件。
有的申請人對所報學校可能不夠瞭解。這樣，如果申請讀本科，就應寫信給Office of Admissions;
如果申請讀研究生，就應寫信給Office of Graduate Admissions.這種寫法的信件肯定能到達承辦
人的手中，稱呼的方法可採用下列二種方式之一：

(2) Office of Admissions Dear Sirs:順便提一下，在選擇學校時，應考慮各種利弊因素。其中包
括地理位置、學科設置、教學品質、學校規模、錄取標準、費用高低、資助條件、住房情況、服務設
施等。

I wish to pursue a doctoral degree in Chemical Engineering at your institution. My desired

General Test this coming October. Would you please send me the application forms for
admission and financial support? Thank you very much.
